M/023/004 THE STATE OF UTAH TO ALL OPERATORS, TAKERS OF PRODUCTION, MINERAL AND ROYALTY OWNERS, AND PARTICULARLY ALL PERSONS INTERESTED IN SECTIONS 32, 33 AND 34, TOWNSHIP 14 SOUTH, RANGE 3 WEST, AND SECTIONS 3, 4 AND 5, TOWNSHIP 15 SOUTH, RANGE 3 WEST, SLBM, JUAB COUNTY, UTAH. Any person or agency aggrieved by this tentative decision to issue approval is hereby requested to submit written protest within thirty (30) days of the date of publication to Mary Ann Wright, Associate Director of Mining, Division of Oil, Gas and Mining, 1594 West North Temple, Suite 1210, Box 145801, Salt Lake City, Utah 84114-5801, setting forth factual reasons for the complaint. If no responsive written protests are received by the Division within 30 days after the last date of publication, the tentative decision of the Division will be final. During Operations: Mining operations include drilling, blasting, loading broken rock, transportation of rock, crushing rock, and rock storage. Limestone, after being pulverized, is blended with other raw materials to produce kiln feed for the manufacture of Portland cement. All mining operations are conducted using conventional mining equipment, including drills, 100 ton haul trucks, dozers, grader, trackhoe, loaders, feeder breakers, belt conveyors and a stacker/reclaimer. The total resource recovery for the life of mine is estimated to be approximately 75 million tons. The quarry life is estimated at 62 years. Approximately 1,200,000 tons of limestone are scheduled to be recovered each year. About 1.1 million tons of waste shale will be disposed of both in a waste pile and in the mined out quarry. The proposed quarrying operation will disturb approximately 118 acres for removal of the limestone and shale. Additionally, other lands will be disturbed for the construction of roads, ponds, and mining facilities. A total of 669 acres are included in the proposed Life-of-Quarry disturbance limit. Soils will only be salvaged from areas where it is practical and can be done in a safe manner due to the steep topography. After Operations: All roads except for the loop that extends north from Highway 132 to the railroad (the north road - 1.2 acres) and a road that extends south from Highway 132 to the quarry (the South road - 4 acres) will be removed, ripped and reclaimed. During the process of mining, a heavy chain will be dragged across the highwall face to remove any loose debris. A three foot high berm will be constructed on the edge of the bench below the highwall to catch any debris. Once the highwall or end wall has been stabilized it will not be disturbed any further. All surface water diversion structures and sediment ponds will be removed and the areas reclaimed. The proposed waste rock stockpile will be constructed at a stable slope of 30 degrees. It will be revegetated with an approved seed mix. All features such as buildings, roads (except for the two discussed above), pads, or other structures will be removed and the areas reclaimed during the final reclamation process. Disturbed areas, except for barren rock highwalls, will be graded, stabilized and have topsoil replaced at a 6" depth, and amended with 5 ton per acre of composted manure. Areas not receiving 6 inches of topsoil will have suitable plant growth materials (fines) applied and amended with ten ton per acre of composted manure. All areas receiving topsoil or other plant growth materials will be seeded in either the fall (preferred) or spring. Disturbance will be ripped to reduce compaction, rough graded to reestablish contours and drainage, and final graded to slopes suitable for the use of mobile equipment for the preparation of seedbeds and revegetation. PUBLIC NOTICE The Juab County Commission will hold a public hearing on November 5 2001 at 6:30 PM in the commission chambers 160 North Main Nephi. The purpose of the hearing is to receive public comment on a proposed revision to the Juab County Subdivision Ordinance. The revision would limited the area in which Cluster Inventive Subdivisions are allowed and place time limitations on subdivision approvals. Copies of the proposed ordinance are available at the office of the Juab County Clerk.
