Monday, August 27, at approximately 11:17 a.m., a fatal crash occurred in Juab County on I-15 near Nephi.
The name of the crash victim is Tuitoto Talataina, age 51, Santa Anna, California.
Talataina, the driver and father of a five-member family traveling together from their home in California to Provo, was killed at the scene, said Utah Highway Patrol Sgt. Hoby Metz.
“The accident involved a family of five occupants,” said Metz. “The other four occupants were treated at Central Valley Medical Center.”
The mother of the family was then transported from Central Valley Hospital to Utah Valley Regional Medical Center in Provo.
Metz said the accident was a single vehicle crash involving a 2000 Toyota Sienna Minivan.
The location of the accident was on I-15 northbound at mile marker 218.
“Evidence at the scene suggests that the driver may have fallen asleep and veered into the median, where the vehicle overturned three complete rolls and came to rest in the inside southbound lane on its wheels,” said Metz.
None of the family members were ejected from the rolling vehicle. All occupants were wearing seat belts.
Talataina sustained head injuries as the vehicle rolled, Metz said. Those head injuries were the suspected cause of death.
“Information received from family members, was that the family was en route to Provo, Utah from Santa Anna, California to enter their son into the LDS Missionary Training Center,” said Metz.
Traffic was rerouted for a time until the victims could be transported and the accident information collected from the scene.
The investigating officer was Utah Highway Patrol Trooper Robert Iverson, said Metz.
