ORDINANCE NO. 5-18-04 ORDINANCE AMENDING SECTION 12-2412 OF THE NEPHI CITY ZONING ORDINANCE WHEREAS, Nephi City has authority to pass ordinances to establishing regulations for land use and development; and WHEREAS, Nephi City Council finds it in the best interest of the City to adopt an amendment to Section 12-2412 of its Zoning Ordinance; IT IS HEREBY ORDAINED, that the following ordinance be adopted and, to protect the welfare of the City, shall become effective immediately. Part 2. of Section 12-2412 shall read: "The site shall abut upon a collector or arterial street shown on the Transportation and Circulation Map of Nephi City, or be within mile of an arterial road with the approval of the Legislative Body." All other provisions of Section 12-2412 shall remain the same. The effective date of this ordinance is the date on which it is passed which is the 18 day of , MAY 2004. public hearing Nephi City will conduct a public hearing at 7:30 pm on Wednesday, June 16, 2004 in the Nephi City Public Safety Building at 42 East 200 North Street. The purpose of the hearing is to accept public comment on the tentative revised budget for fiscal year 2003-2004 and on the tentative budget for fiscal year 2004-2005. Copies of the tentative budgets will be available for public inspection at the office of the Nephi City Recorder. PUBLIC NOTICE The Department of Air Quality will take public comment regarding their proposed Approval Order for John Kuhni Sons new Animal Rendering Plant, located at 6480 West Mills Road, at a hearing starting at 7 P.M. June 17th at the Levan Town Hall, 20 North Main, Levan, Utah.  John Kuhni Sons must comply with detailed requirements, limitations and conditions outlined in the proposed Approval Order.  The proposed Approval Order is for the installation of a new animal rendering plant.  This plant will produce a variety of animal products and grease.  A full copy of the order is available at the Division of Air Quality, Utah Department of Environmental Quality, 150 North 1950 West, Salt Lake City. All comments will be evaluated before the Division of Air Quality issues a final approval order.

PUBLIC NOTICE The Juab School District Board of Education will conduct a public hearing on Wednesday, June 16, 2004, at 6:00 P.M. at the Juab School District Office to receive public comment on the revised budget for fiscal year 2003-2004; and on the tentative budget for fiscal year 2004-2005. PUBLIC NOTICE OF AMBULANCE LICENSE APPLICATION As required by Utah Code § 26-8a-405, notice is given that Eagle Mountain Fire Department has applied for a new license to provide paramedic rescue service. The proposal is for a Paramedic Rescue License to be operated by Eagle Mountain Fire Department, with a variance to allow Eagle Mountain to staff one Paramedic and one Intermediate from 0600 to 2200 until July 31, 2005. Eagle Mountain has an interlocal agreement with Lone Peak to respond to calls according to medical priority during the hours of 2200 and 0600 and will provide back up when requested. Eagle Mountain will then staff one Paramedic and one Intermediate 24 hours a day, until July 31, 2006. Eagle Mountain will then staff two full time Paramedics thereafter. Eagle Mountain will provide paramedic rescue service in the following geographic area: Beginning at the intersection of Utah, Tooele, and Salt Lake Counties; then East along the Utah/Salt Lake County boundary until the boundary line intersects the eastern boundary line of Section 5 of Township 5 South, Range 1 West; then South along the eastern boundary lines of Section 5, 8, and 17 of Township 5 South, Range 1 West to the intersection of State Road 73 and the current eastern boundary of the Town of Eagle Mountain; then South along the current eastern boundary of the Town of Eagle Mountain and continuing southward along the current eastern Eagle Mountain Town boundary to the southeast corner of Section 6 of Township 7 South, Range 1 West; then South along the eastern boundary line of Sections 6, 7, 18, and 19 of Township 7 South, Range 1 West to the intersection of Soldiers Pass Road; then east along and including all of Soldiers Pass Road to the intersection of Lake Mountain Communications Road; then due South to the intersection of the northern boundary line of Section 29 of Township 7 South, Range 1 West; then east to the northeast corner of Section 29 of Township 7 South, Range 1 West; then South along the eastern boundary lines of Sections 29 and 32 of Township 7 South, Range 1 West to the southern boundary line of Township 7 South Range 1 West; then West along the southern boundary lines of Township 7 South Range 1 West, 2 West, and 3 West until the Utah/Tooele County boundary; then North along the Utah/Tooele County boundary until the point of beginning Parties defined as interested parties under the EMS act may object to the application in writing by June 17, 2004 until 5:00 p.m. to the Bureau of Emergency Medical Service, P.O.
