REQUEST FOR PROPOSAL Six County Association of Government Request for Proposal The Six County Area Agency on Aging is accepting RE QUESTS FOR PROPOSAL for the Registered Nurse component of the. Aging Medicaid Waiver program. The Waiver program requires an RN component in assessing frail clients in need of in-home support in lieu of placement in it nursing home. The RN will work its an assessment team member with a Case Manager, The RN component will be required for approximately 35-40 assessments per year and will be a part of an assessment when a licensed medical opinion is evident. The RN portion of an assessment is on an on call basis Proposals are to be submitted for an hourly rate for nursing service, travel will be reimbursed for actual miles incurred for each assessment.
